As alumni and friends of the Frances Payne Bolton School of
Nursing, you are invited to nominate FPB alumni as candidates
for consideration to receive one of the prestigious FPB Alumni
Association Awards listed below. A list of past recipients of the Distinguished Alumni Award and Award for Excellence is available here, and information on our 2007 recipients can be found here.
Distinguished Alumni Award

The highest recognition given by the Alumni
Association. Eligible alumni include those who have demonstrated
continuous, outstanding, creative and exemplary contributions
to the discipline of nursing or to healthcare or have made
noteworthy contributions in other fields. |
Award for Excellence
| |
Recognizes alumni who have made significant
contributions in nursing or related areas of healthcare.
Candidates must have established careers that show evidence
of meritorious contributions to the discipline while promoting
a standard of practice worthy of the Award for Excellence. |

These awards will be presented on Alumni Celebration
2008 Weekend during the Alumni Luncheon on Saturday, May 17,
2008. |

Criteria for Candidates
|
| |
1. |
Alumna/us of the Frances Payne Bolton School
of Nursing. |
| |
2. |
Involvement in some phase of nursing or healthcare or
noteworthy contributions in other fields of endeavor. |
| |
3 |
Distinguished Alumni Award |
| |
|
Outstanding leadership/achievement in nursing and/or healthcare,
characterized by noteworthy contributions in two or more
of the following: clinical expertise; administration; education;
scholarship (writing & research); consulting; professional
leadership; community service; entrepreneurial endeavors;
public relations and humanitarian activities. |

|
Award for Excellence |
| |
|
Meritorious contributions reflecting creative or innovative
approaches in one or more of the following areas of expertise:
clinical/patient care; program/institution administration;
academics/education. Candidates will be evaluated in areas
of scholarship (research, writing, publication), leadership,
community work, consulting and entrepreneurial endeavors. |
Requirements for Nomination

Please submit a letter of nomination for
your candidate that includes the title of the award for
which you are nominating the alumna/us and describes how
your candidate's career activities meet the specific criteria
for that award. You must also include a copy of the candidate’s
curriculum vitae and contact information. Please also send
us your personal contact information (name, daytime phone
number, and address) should we need to reach you. (Nominators
need not be alumni of FPB School of Nursing.) Please mail
your entire packet of materials by February 7, 2008
