Policy on Health Requirements
All BSN students must maintain current documentation on file with the University Health Service and the BSN Program that they meet certain health requirements set by our clinical agencies (see chart and instructions below).
Students may not attend any clinical experiences throughout the program unless documentation is on file that all health requirements are met. Records may be checked at any time.
Procedure for Providing Required Documentation
Visit the University Health Service website and download the NEW STUDENT MEDICAL PACKET. Print the forms, read the instructions, determine what health services you need, and begin completing the requirements no later than June 1, 2009.
Mail all required health information to University Health Service no later than August 1, 2009:

The data will be entered into your University Health Service records. You will be able to see the data, and it will indicate whether you comply with each requirement. It may take up to two weeks before your records will be available on that website. You must have activated your network ID to log into the website; see our instructions.

Titer: A titer is a blood test to measure immunity to an infectious disease. A report that the student had the disease in the past is not sufficient.
To begin clinical experiences, at least two doses of each vaccine (or positive titer) are required. The third dose of Hepatitis B and titer must be completed according to the prescribed schedule, which is approximately five months after the second dose. The third dose can be administered by Health Services for the cost of the vaccine and titer.

The University Health Service cannot legally provide services to new students until classes start, and they are very busy at that time. Call early for appointments should you need to be seen by a health care provider.
