The state report may be of the state of residence at the time of the background check.

Policy: Students must have acceptable reports of state and FBI background checks by fingerprint sent to the Graduate Entry DNP Program (see below) in order to attend any clinical experiences. Digital fingerprints are strongly recommended. |

The reports must be received by the Graduate Entry DNP Program no later than Friday, August 22. |

NOTE: Students who apply for RN licensure in Ohio and most other states will have to have another background check done at their expense during the 4th semester of the program (report must be less than one year old). Instructions may be found on the Ohio Board of Nursing Website. We will have National Background Checks, Inc. come back to FPB at an appropriate time in the Fall, 2009 semester. |
