Health and Safety Requirements

Overview of Requirements
All students must maintain current documentation on file with the University Health Service and the Graduate Entry Program office that they meet the health & safety requirements set by our clinical agencies.
Students may not attend any clinical experiences during the program unless their health & safety records are current. Clinical experiences for Graduate Entry students begin early in the semester.
There are four types of requirements. Please read the sections below for detailed information on each requirement:

Students are responsible for assuring their records are current at all times.

Due to confidentiality policies, only the student can view and download their health record.

Be sure to keep personal copies of all your health records.

Health records are sent to University Health Service. Only your Health Service report is sent to the Graduate Entry Program office.

Policy on Health Requirements

All Graduate Entry students must maintain current documentation on file with the University Health Service and the Graduate Entry DNP Program that they meet certain health requirements set by our clinical agencies (see chart and instructions below).

Students may not attend any clinical experiences throughout the program unless documentation is on file that all health requirements are met. Records may be checked at any time.
Procedure for providing required documentation to the University Health Service and the Graduate Entry Program:

NOW! Go to the University Health Service website and download the medical packet. Complete the forms and return them to the University Health Service as soon as possible.

Read the instructions, determine what health services you need, and complete them.

Do not expect to get the vaccines, titers or TB skin test #1 required to start clinicals at University Health Service. They do not see new patients before classes begin and after that there is not enough time. You can get TB skin test #2 at no cost on Friday, August 26.

By August 1: Submit all required health information to the University Health Service.

University Health Service will enter your health information in their database. You will be able to see the data on https://myhealthconnect.case.edu, which will indicate whether you comply with each requirement.

Disregard the compliance listing on TB – it will say you are OK with one skin test; however, you must have two.

You must also have a varicella titer, regardless of what the form says.

By August 15: Go to myhealthconnect.case.edu, log in, and download a copy of your health report showing compliance with all requirements. You must have activated your network ID to log into the myhealthconnect website; see instructions under Registration.

Give or email a copy of the report to Anne Watson, Graduate Entry Department Assistant. She will send you an email confirming your compliance.

You must give Anne Watson an updated copy of your Health Record every time you have an additional skin test, vaccine or titer. Due to confidentiality laws, we cannot view or print your record.

2 step TB test required to begin clinicals at start of program. After the initial 2-step TB test, a single annual TB skin test is required. If you have a positive skin test already – contact University Health Service. |

Urine Drug Screen (new in 2011) |
This test may be required by specific clinical sites students are assigned to throughout the MN, MSN, DNP programs. |
Students will be informed asap of the requirement and how it will be done. Each student is responsible for the cost of their test. |
Titer: A titer is a blood test to measure immunity to an infectious disease. A report that the student had the disease in the past is not sufficient.

Declination statement: A signed declination statement may be submitted in place of immunizations. A copy of the form is downloadable from the Health Services website.
Warning: Arrange to complete these requirements before starting classes. The University Health Service cannot legally provide services to new students until classes start and they are very busy at that time. You may need to wait a week or two for an appointment and longer to receive blood test results. This delay will prevent you from attending clinical experiences.
NEONI Orientation Study Guide and Competency Exam

All students from NE Ohio schools of nursing must annually pass a competency exam with grade of 90% or higher before attending ANY clinical experiences in any area hospital.

The exam covers basic but essential information on topics such as safety, infection control and protection of confidentiality. It is based on the NEONI (Northeast Ohio Nursing Initiative) Orientation Study Guide.

Further information will be provided during summer after we receive the Study Guide and exam for 2010-2011.

Students will be required to read the guide and take the test on Blackboard no later than Monday, August 22.
