Documentation of all of the above must be provided to the Department Assistant prior to beginning clinicals.
We need to have a copy of your official report from BCI regarding criminal background check (fingerprinting). Fingerprinting can be arranged at University Protective Services. You need to make the arrangements on an individual basis.
To help you complete the fingerprint process quickly, please bring your university ID, any documentation requesting fingerprinting, and a drivers' license or state-issued ID.
Fingerprinting can also be arranged through National Background Check Incorporated (877-932-2435).
Cost for Ohio is $35; for national, $55.
