We are seeking FT, PT, PRN or float nurses to function independently in providing comprehensive care to patients (living in their homes) who are experiencing mental illness - both acute and chronic disease states.
Requirements include:
- RN in the state of Ohio
- experience in community health nursing
- minimum 3 years psychiatric/mental health nursing experience
- home care experience and venipuncture skills preferred
- reliable transportation

Responsibilities include:
- utilize nursing process to gather database, plan and evaluate nursing care
- initiate health care plan
- demonstrate clinical expertise in providing care to patient
- initiate referrals with other team members and community agencies
