The Center for Self-Management Advancement through Research and Translation (SMART) Center at Case Western Reserve University's Frances Payne Bolton School of Nursing will sponsor a Self-Management Conference, On the Road to Self-Management of Health: Many Paths, on Monday, October 17, 2011.

Kate Lorig, RN, Dr. P.H., Professor in the Department of Medicine and Director of the Patient Education Research Center from Stanford University, will be the keynote speaker. Conference speakers will address the application of models and concepts of self-management in multiple populations in varied environments of care.
