All BSN students must maintain current documentation on file with the University Health Service and the BSN Program that they meet certain health requirements set by our clinical agencies (see chart and instructions below). |

Students may not attend any clinical experiences throughout the program unless documentation is on file that all health requirements are met. Records may be checked at any time. |
Procedure for providing required documentation to the University Health Service and the BSN Program |

NOW!! GO to the University Health Service website & download the NEW STUDENT MEDICAL PACKET print the forms, complete them, then mail them to University Health Service as soon as possible. |

By June 15: Read the instructions, determine what health services you need and complete them. |

Before August 1: Submit all required health information to the University Health Service |

University Health Service will data enter your health information. You will be able to see the data on https://myhealthconnect.case.edu and it will indicate whether you comply with each requirement. |

On or Before August 15: Go to myhealthconnect.case.edu, log in, and download a copy of your health report showing compliance with all requirements. (You must have activated your network ID to log into the myhealthconnect website; see instructions on a separate section of the New Student Website.) |

Mail or email a copy of the report to Marla Radvansky, BSN Department Assistant. She will send you an email confirming your compliance.

This requires two skin tests and two readings (4 visits).
The 2nd skin test must be done within 12 months of the first one. |

Both skin tests may be completed by a non-CWRU provider prior to August 15.
If one skin test has been done, the 2nd test may be done at the School of Nursing at no charge during orientation or the first week of classes (watch for notices of TB testing at FPB).
TB testing each year after entering the program may be done on campus at no charge. |

Titer: A titer is a blood test to measure immunity to an infectious disease. A report that the student had the disease in the past is not sufficient. |

To start clinicals: To begin clinical experiences, at least two doses of each vaccine (or positive titer) are required. The 3rd dose of Hepatitis B and titer must be completed according to the prescribed schedule. Students should update their records in the BSN department office (#1550) as they receive additional doses of vaccine. |

Declination statement: A signed declination statement may be submitted in place of immunizations. A copy of the form is downloadable from the Health Services website. |

Warning: Arrange to complete these requirements before starting classes. The University Health Service cannot legally provide services to new students until classes start & they are very busy at that time. You may need to wait a week or two for an appointment and longer to receive blood test results. This delay will prevent you from attending clinical experiences. |
